ترامادول یسک ضد درد صناعی موثشر بر سیستم اعصاب مرکزی است که کپسول ‎٩۰‏ میلی گرمی؛ آمپولهای یک و دو میلی لیتری و قرص ترامادول رتارد(آهسته رهش) ۱۰۰ میلی گرمی آن توسط شرکت ستاعتی گنجیداون توایوشده است:

* آمپول او ۲ میلی لیتری به ترتیب حاوی ۵۰ و ۱۰۰ میلی گرم. کپسول آن حاوی ۰۰ میلی گرم و قرص آهسته رهش حاوی ۱۰۰ میلی گرم ترامادول می باشد.

* ترامادول و متابولیست فعال آسن آگونیست رسپتورهای اپیوئیدی از نوع ل| هستند. به علاوه فعالیت مونوآمینرژیک ترامادول در مهار بازجذب نورونسی نوراپسی نفرین و سروتونین می تواند با بلوک ایمپاللس های دردناک, در خاصیت ضد درد این دارو نقش داشته باشد.

* مسیر اصلی متابولیسم این دارو کبدی و وابسته به سیتوکروم 60۰- 7بوده و 1۰ دارو پس از متابولیزه شدن از طریق ادرار دفع می شود. * ترامادول از جفت عبور کرده و در شیر مادر ترشح می شود.

در این خصوص ذکر اين نکته ضروري است: * فرم کپسول و قرص ترامادول در کودکان زییر ۱۶ سال و فرم تزریقی در کودکان زیر یک سال نباید تجویز شود.

در بیماران 1 سنال بةبالا:۵۰ نا ‎٩۰۰‏ میلی گرم تراماذول با توجه به شدت درد به عنوان دوز اولیه تجویز می شود که در فواصل ۶ تا * ساعت قابل تکرار است(البته قرص آهسته رهش حداکثر دو بار در روز تجویز می شود). حداکثر مقدار داروی مصرفی ۰۰ میلی گرم در روز است.

؟ اطلاعات موجود برای افراد 70 تا ۷۰ ساله بدون اختلال کبدی یا کلیوی» تغییر دوزی را پیشنهاد نمی کند. ولی در سنین بالای ۷۰ سال مجموع دوز مجاز به کمتر از ۳۰۰ میلی گرم در روز کاهش یافته و دوزهای منقسم توصیه می شود.

نحوه مصرف در نارسایی کلبه : در افراد دچار نارسایی کلیه. دوز مجاز حداکثر ۲۰۰ میلی گرم و فاصله زمانی مناسب جهت تجویز دارو هر ۱۲ ساعت برای بیماران با کلیرانس کراتینین کمتر از 5960 ۲۳۷/۲۳۸ می باشد.

* نکته: اثر تضعیف کنندگی سیستم تنفسی, در فرم خوراکی ترامادول به مراتب کمتر از سایر اپیوئیدها دیده می شود.

موارد احتیاط و منع مصرف دارو : * ترامادول در موارد حساسیت به اين دارو یا واکنش های آنافیلاکتوئید نسبت به کدئین یا دیگر اپیوئیدها نباید استفاده شود. * ترامادول در موارد افزایش فشار داخل جمجمه یا ضربه مغزی, موارد شکم حاد, نارسایی تنفسی» کبدی و کلیوی و وابستگی به اپیوئیدها با احتیاط تجویز می شود. * ترامادول خوراکی در کودکان زیر ۱۶ سال و ترامادول تزریقی در کودکان زیر یک سال توصیه نمی شود.

موارد احتیاط و منع مصرف دارو : * ترامادول در دوره بارداری (گروه () حاملگی می باشد) شیردهی. در بیماران دچار صرع و بیماران با ريسك بالا برای حملات تشنجی توصیه نمی شود.
